    Abstract

    Introduction: Today, with the development of medical science and its related technology, cancer in children and adolescents has gradually turned from an acute and fatal disease into a chronic disease and the survival rate has increased. The aim of the treatments used today for children with cancer is not only to increase the survival rate but also to increase the quality of life of these children. As a result, it is important to examine the factors related to the quality of life of children with cancer.

    Purpose: The purpose of this study is to determine the factors related to the quality of life of children with cancer.

    Methodology: This research is a descriptive-analytical cross-sectional study in which 213 children aged 2-14 years with cancer referred to Imam Reza Clinic (AS). and Amir Shahr Shiraz Hospital participated. The data in this research was collected using a questionnaire consisting of three sections: 1) factors related to the quality of life of children with cancer and the general health of mothers, 2) health-related quality of life (TNOAZL) and 3) specific quality of life (Ped QL Cancer Module) of children with cancer through interviewing the mother and observing the child's file. After collecting the data, it was entered into SPSS software version 19. The results were analyzed using t-test, one-way analysis of variance, Pearson, Mann-Whitney, Kruskal-Wallis correlation coefficient, Spear-Man correlation coefficient and multiple logistic regression. Results: The results showed that the average score of health-related quality of life was (61.38±12.34), specific quality of life was (64.61±16.83). The examination of individual-social and clinical factors and the general health of the mother showed that, based on the multiple logistic regression model, the age of the child, the duration of the disease, the interval between the chemotherapy courses and the number of radiation therapy courses with the quality of life related to the health and age of the child, the general health of the mother, the interval between the chemotherapy courses and the number of radiation therapy courses had a statistically significant relationship with the specific quality of life (p<0.05).

    Conclusion: Considering that among the factors related to the quality of life related to the health and specific of children, the age of the child and the general health of the mother, the duration of the disease, the interval between chemotherapy courses and the number of radiation therapy courses are most related to the quality of life of children, therefore it is very important to pay attention to the quality of life of all children and their families, especially in the aggressive period of treatment, when they are more vulnerable. Keywords: cancers, children, quality of life Its physical dimension has been taken into consideration and less has been mentioned about its mental and social health dimensions. Health means the ability to adapt, effective communication, responsibility, good nutrition, good physical performance, stress management, appropriate mental state and effective social performance (1). The World Health Organization[1] considers "health to be a state of physical, mental and social well-being and not just to improve the disease" (2). Today, health diagnosis and evaluation of health interventions pay attention to other human values ??such as quality of life in addition to calculating mortality indicators, frequency and severity of disease(4). Quality of life is a broader concept of health that includes a set of physical, mental and social well-being (5). In fact, the quality of life has a multidimensional structure that is examined in different functional, social, psychological, and emotional dimensions (6). Chronic diseases are among the disorders that severely affect people's health and, as a result, their quality of life (8).Chronic disease is a condition that disrupts daily activities for more than 3 months per year or causes hospitalization for more than one month per year or at the time of diagnosis. The development of technology and drug treatment has increased the number of children suffering from chronic diseases (9). Approximately 10-40% of children in the world suffer from chronic diseases (10). The impact of chronic and debilitating diseases on a child's performance and health status is very high. These children need 3 times more bed rest than other children, 2 times more need a doctor and 5 times more hospitalized than other children. Chronic illness may last for years or continue throughout life and affect all activities of children and parents. Among chronic diseases, cancer is one of the things that has always attracted the human mind (9).

    Cancer is a general term for complex diseases that arise as a result of some genetic changes and cause uncontrollable and irregular cell proliferation (11). The term pediatric cancer [2] refers to cases of cancer that are diagnosed in children under 15 years of age (12). Children's cancer includes a group of malignancies, each of which has different epidemiological characteristics, biological characteristics, treatment methods, and survival problems (13). Cancer creates unfavorable physical, psychological, social and economic conditions for the family and society, and causes many years of the patient's life to be spent coping with the disease and pursuing treatment methods (12).  As the death rate of childhood diseases (especially infectious diseases) has decreased in industrialized and developing countries, the number of children diagnosed with cancer has increased (14). In Europe and America, this disease has been reported as the main cause of death in children aged 5-14, and its prevalence in this age group is approximately 129 per million people (9). The survey conducted by Mahek (Institute for the Support of Children with Cancer) regarding the prevalence of cancer in Iran shows that the number of children with cancer was 9 out of every 100,000 children per year in different years in the country, which increased to 15 children in 2008 (4).  According to the report of the Fars Province Cancer Registry Center, during the years 1380 to 1387 (for 8 years), the number of children with cancer was 1610, among which blood cancers were the most frequent (47.8 percent), followed by brain tumors and other parts of the central nervous system (9.5 percent) and lymph nodes (9.3 percent), which accounted for the most cases (15).

    The quality of life of the sick child is jeopardized as a result of the pain caused by the disease itself and aggressive treatment methods, as well as a decrease in energy to enjoy normal daily activities, the inability to establish contact with friends at school and in the community, and fear of the future (20). Treatment methods make the child tired and sick all the time. These children are also susceptible to infection and are continuously admitted to the hospital, which leads to the separation of the child from the family, school and community (21). Cancer treatments are accompanied by complications and toxicity that endanger the quality of life of a person in the short or long term (6). Treatment methods such as chemotherapy, radiation therapy, or surgery may have adverse effects on childhood development and cause long-term physical, psychological, and social effects in childhood and adulthood (22).
