Abstract:
Background and purpose: Infertility imposes many psychological and economic effects on the health system of society. It is obvious that the timely diagnosis and treatment of varicocele as the most common cause of treatable infertility in men can play an effective role. Materials and methods: This study is a cross-sectional study, the data of which was collected from among 150 infertile patients who referred to the Jihad Infertility Center of Qom University between 1987 and 1990 in a random manner and by completing a questionnaire and calling the patients, and then the information was obtained. They were entered into SPSS V.16 software.
Findings: There was no significant relationship between the fertility rate after varicocelectomy with age, body mass index, type of infertility and preoperative semen analysis of the patients in our society, but this relationship was significant in the grade of patients who had the highest fertility rate in grade 1 and in total 28.03% of the patients became pregnant.
Conclusion and suggestions: The reason for the lower fertility rate after varicocelectomy in our society can be related to the infertility issues of the patients' wives, our shorter follow-up period, and the patients' lack of cooperation in completing the questionnaires. Therefore, another analysis of the patients should be performed after the operation of a semen analysis, and the rate of recovery was investigated.

Introduction:
Infertility imposes many psychological and economic effects on the health system of society. . It is obvious that timely diagnosis and treatment of varicocele can play an effective role.
The existence of dilated and twisted veins of the Pampini network in the form of spermatic cord is called varicocele (6), which is found in approximately 15% of young men (6, 7). This disease is the most common cause of male infertility (7,8). Some researchers believe that only symptomatic varicoceles should be treated (7). There is a lot of evidence that varicocele reduces the quality of semen in people and causes a decrease in fertility (6-7, 9) and on the other hand, it is clearly related to the decrease in the volume and consistency of the testicles (5, 6) and its treatment improves sperm parameters and the incidence of pregnancy (8-9, 12) and even improves the sexual activity of patients (13).
Varicocele is the most common surgically correctable cause for reduced fertility, the first line of treatment for varicocele is varicocelectomy, but every varicocele does not require surgery. Indications for surgical correction are testicular discomfort and pain (which cannot be relieved by symptomatic treatment), testicular atrophy, reduction in the quality of semen, and the possibility of contributing to unexplained infertility in men.
Among the most important indications for varicocele correction are abnormality of semen and smaller testicles (6). In cases where the patient has reduced fertility, if the semen test shows Stress Pattern, varicocelectomy will be indicated.
Various medical and surgical methods are used to treat this disease, which surgical methods include: retroperitoneal, laparoscopic, inguinal, subinguinal, and suprainguinal ligations.
Another treatment method for this problem is medical treatment, whose role on varicocele is still not well known.Some researchers are evaluating the role of antioxidants on the treatment of varicocele, so this treatment of varicocele is still experimental. The use of HCG in men with less than ten million sperm count is another treatment. HCG stimulates the production of testosterone in Leydig cells, therefore, the administration of HCG can be useful in certain patients with varicocele and low sperm count and dysfunction of Leydig cells. But this method is still under discussion.
It has also been found in various studies that varicocelectomy increases the success of ART (Assisted Reproductive Technique) methods and even changes in different levels of ART use (14 and 15). Considering the role of varicocele in causing a harmful effect on sperm parameters and reducing the pregnancy rate, the effect of its treatment with varicocelectomy on its recovery, this study was conducted to investigate the effect of varicocelectomy on the incidence of pregnancy. rtl;">
