    Due to the very weak coverage of basic treatment in the dental insurance sector, it has caused such services to be considered as luxury services for people and the health indicators of oral and dental health in Iran are not very desirable. With regard to the coverage of supplementary excess treatment by commercial insurances, in this research, the influencing factors on the use of dental services were investigated. According to the definition of applied research, this research is applied, and considering the way of collecting data and describing what it is, the research method is a survey, in this research, the information is collected through a questionnaire and using descriptive and inferential statistics methods, one-population and binomial t-tests, and Friedman was analyzed.

    Kalamro is the place of research of Iran Insurance Company (in Tehran). The time of investigation in this research was carried out in 2012.

    The community studied in this research is dental insurance insurance of Iran Insurance Company in Tehran city, whose number is 900,000 people. According to the size of the studied population, the sample size is determined to be 384 people. The samples were selected by stratified random method. In this research, we examined some aspects affecting the use of dental services according to the research model. Also, these dimensions have been tried to be ranked in terms of importance. Key words: dental insurance, DMFT, access to dental services 1- Introduction Basic insurance and supplementary insurances in the country currently do not cover restorative and specialized dental services, but insurances such as army insurance, banks, shipping and some other organizations or ministries provide services in this field in the form of a new definition. covered. Justice finds the right definition when all the people of the country are covered by dental services insurance in the form of basic insurance, but currently, in terms of economic power, such a thing is far from the power of basic insurance.

    On the other hand, the improper distribution of dentists at the country level and the limited specialized services in small towns, villages and deprived areas cause a rural person to pay an insurance premium equivalent to someone living in the city. This issue distorts the pricing of dental services.

    The most important indicator of oral health in Iran and the world is the (DMFT) index.  which shows the amount of decayed, extracted and filled teeth in different age groups. In the country, last time in 1983, this index was measured nationally for the 12-year-old age group, and it showed that every 12-year-old Iranian has an average of 1.86 rotten, filled, or extracted teeth.

    In developed countries, people at the age of 80 should have at least 20 healthy teeth. In these countries, at the age of 35 to 40, all teeth are kept healthy. Unfortunately, this is despite the fact that according to the statistics of the Department of Oral and Dental Health of the Ministry of Health, the average number of healthy teeth among Iranians is between the ages of 21 and 21.7. In fact, the result of pulling teeth as a treatment solution ends in the problem of needing dental prostheses at the age of 40 to 50 in Iran. On the one hand, insurance does not pay for services such as tooth filling or denervation, on the other hand, the cost of these things in dentists is at least one-third or half of the salary of a worker or employee, and finally, with the help of insurance, only tooth extraction is included in insurance services. Meanwhile, the relationship between oral and dental health has been proven with other parts of the body, for example: weight loss is related to gum health, good chewing ability is related to the risk of dementia, dental scaling is related to reducing heart attacks and other issues.    

    1-2- Statement of the problem

    Basically, insurance as a commodity and as an asset can be analyzed based on the microeconomics method. Considering that related theories generally introduce insurance as a normal product, it is expected that the level of demand for medical insurance will increase with the increase in income.     Accurate evaluation of the characteristics of the insurance market is one of the important activities that should be carried out in order to create the necessary knowledge, to design and regulate development policies. Teeth in Iran are not very desirable. With regard to the supplementary excess treatment coverage by commercial insurances in this section, the examination of the use of dental insurance and its effect on oral and dental health indicators is considered the main source of this research.

    In this section, while examining the supplementary dental treatment packages in the contracts set up since the beginning of such insurances, it examines the factors affecting the use of dental services that cause the improvement of oral and dental health indicators. The main goal of this research is to investigate the amount of affordability resulting from the purchase of supplementary treatment insurances on the use of dental services according to the different characteristics of the insured and the effect of the indicators of the increase in oral and dental health insurance according to the amount of affordability offered.

    A) The urgent need of commercial insurance companies to include in the supplementary package Dental treatment with a high weighting factor. b) The increasing demand of insurance customers for dental services in excess treatment insurance packages from insurance companies, and providing a suitable response to these requests. c) The importance of managing the risk of dental excess insurance in commercial insurance companies and preventing the losses of these companies. Treatment. 1-5- The theoretical framework of the research. The theoretical framework of the research is the attitude or perspective on which the entire research is based, and at this stage a decision is made regarding the rejection or acceptance of the problem raised in the hypotheses. In this research, the amount of use of dental services is used as a dependent variable to identify and measure it from the research done by Moonkyung Kate Choi and published his research results in 2011 in the Journal of Health Economic. He has used dental services. On the other hand, having dental insurance coverage, having children, and the observable characteristics of the person, which are: marital status, gender, education level, and DMFT index, have been considered as independent variables.

    In this research, in addition to the mentioned independent variables, we added the access variable to the independent variables according to the opinion of experts in this industry, and then we measured their impact from the point of view of dental insurance policyholders in Iran Insurance Company.

    In this research, an attempt has been made to examine the effective factors in increasing the use of dental services in Iran, and by introducing it to experts, decision makers and decision makers in the field of insurance, to help identify the potential and actual capacities of the country's insurance industry.
